
A bichectomy, also known as buccal fat removal, is a cosmetic surgical procedure aimed at reducing the volume of the buccal fat pads in the cheeks to create a more contoured facial appearance. This procedure is popular among individuals seeking to achieve a more defined facial structure, particularly in the cheek and jawline areas. Buccal fat pads are encapsulated fat masses located in the middle part of the cheeks between facial muscles, beneath the cheekbone area. Everyone is born with buccal fat pads, but their size varies significantly from person to person. In some individuals, large buccal fat pads can create a “baby face” appearance with rounded cheeks. A bichectomy is typically performed under local anesthesia, sometimes combined with sedation, and takes about one hour. The surgeon makes a small incision, about 2 to 4 cm, inside the mouth on the inner portion of the cheek. Through this incision, the surgeon accesses and extracts a portion of the buccal fat pad. The amount of fat removed depends on the desired outcome and the original size of the fat pad. After the removal, the incision is sutured closed.
